New Hire Forms

Welcome to the new hire process at Lapeer Community Schools! 

As part of the new hire process, all employees will participate in a new hire meeting. During the meeting, you will be provided with several employment-related documents. If you wish to print and complete some of the forms ahead of time, click on the link below to get started. You may complete these forms and print or email copies in advance of the meeting. 
 
Tax Forms

All new employees are required to complete the Federal, State, and City W-4 as well as the I-9 form and provide documentation of identity and employment authorization.

EEO Reporting Form 

Lapeer Community Schools is subject to certain governmental recordkeeping and reporting requirements for the administration of civil rights laws and regulations. In order to comply with this requirement, we invite employees to self-report. Submission of this information is voluntary and refusal to provide will not subject the employee to any adverse treatment.

Unprofessional Conduct Form 
Pursuant to State Law, all new employees hired by a K-12 school district must have an unprofessional conduct check completed. New hires must authorize the district to contact their two most recent employers.

Criminal Records Release Form 
Pursuant to the National Child Protection Act (NCPA) of 1993, prior to beginning work in a K-12 educational setting, your criminal history must be reviewed. In order to complete this necessary step in the employment process, new hires must authorize the district to receive the results of a criminal record check.

Direct Deposit Form

Employees have the option of having paychecks direct deposited into a bank account or receiving pay on an ADP Debit Card.

A copy of the entire new hire medical benefit guide can be found at the link below. This information will be reviewed during the new hire meeting and forms may be returned at a later date. Specific information regarding employee contribution cost for all available medical plans can be found on page 3 of the guide.
